Patient resources at Sealy AIC

Patient Resources

Everything you need to prepare for your infusion therapy — forms, insurance information, the referral process, and answers to common questions.

The Referral Process

Infusion therapy at Sealy AIC requires a physician referral. Here is what to expect from the moment your physician refers you to the start of your treatment.

01

Physician Submits Referral

Your physician or specialist submits a referral and prescription for infusion therapy to our clinical team. We accept referrals by fax, phone, or through our referral form.

02

Insurance Verification

Our team contacts your insurance provider to verify your benefits and determine coverage for your prescribed therapy. We will notify you of any out-of-pocket costs before your first appointment.

03

Prior Authorization

For many infusion therapies, prior authorization from your insurance is required. Our staff handles this process on your behalf and coordinates with your physician as needed.

04

Scheduling Your Appointment

Once authorization is obtained, we will contact you to schedule your first infusion appointment at a time that is convenient for you.

05

Begin Treatment

Arrive at our infusion center where our clinical team will review your treatment plan, answer any questions, and administer your therapy safely and comfortably.

Patient Forms

To help streamline your first visit, please review the following information. Our staff will provide the necessary forms at check-in or can send them to you in advance — contact us to request forms before your appointment.
